TWI has developed a planning process to help home health care executives evolve their people into a team with a common, unified focus . . . a focus on gaining and sustaining competitive advantage in a constantly changing home health care environment. The TWI strategic planning process is broken into three phases.
 

Phase I: Information Gathering

All good plans are based on solid information and common agreement as to the starting point of the planning activity. To begin your planning activities, Tweed-Weber will design and implement a customized, information-gathering process utilizing a combination of any or all of the following activities:

  • community positioning study
  • market opportunity assessment
  • referral source surveys
  • employee focus groups
  • board of directors retreat
  • organizational assessment


Armed with this information, your management team will develop a consensus as to the starting point of the planning process. During this activity, the TWI facilitator will help your team

  • analyze the state of your organization, its strengths and weaknesses, as it exists today.
  • define your organization's opportunities and threats, now and into the future.
  • identify forces and trends affecting the home health care industry.
  • identify the key issues facing your organization as it plans for its future.
     

Phase II: Planning

The planning activities in Phase Two are divided into two segments: Strategic Planning and Annual Operational Planning. In each of these segments, the TWI facilitator will lead your planning team through a series of steps that will enable them to

Strategic Planning

  • identify and select unique sources of competitive advantage on which your organization will base its future strategic direction.
  • create/modify/affirm the mission of your organization.
  • develop a vision statement for your organization.
  • develop strategies and action steps that will convert your organization's vision into reality.
  • identify key measures to monitor progress toward your organization's strategic plan.


Annual Operational Planning

  • select first-year operational objectives and assign accountability to specific individuals within your organization.
  • develop action steps and a time line for each objective.


Phase III: Document Preparation and Communication

Phase Three provides your team with the opportunity to review the planning process and collectively commit to your plan's successful implementation. It also allows effective communication of the plan to those identified as critical to your organization's success. Specifically, the team will

  • review the document and finalize its content.
  • define activities to communicate the plan to all identified stakeholders.
  • develop a formal process for monitoring the ongoing implementation of the plan.

Once finalized and communicated, your strategic plan will enable your organization to proactively maximize the opportunities that exist within the dynamic home health care marketplace.
